BEFORE COMMENCING RESIDENCY TRAINING AT THE UNIVERSITY OF KANSAS MEDICAL CENTER, RESIDENTS WISHING TO VISIT KUMC SHOULD SUBMIT THE FOLLOWING DOCUMENTATION:
- Letter from Visiting Resident to preceptor (Program Director) requesting the rotation and outlining goals and objectives. Include dates of rotation.
- Letter from Visiting Resident’s current Program Director (external) addressed to preceptor (internal Program Director) expressing support of this rotation. Letter must include dates of rotation and a statement that Visiting Resident is in good academic standing.
- Letter from preceptor (Program Director) to University of Kansas Medical Center DIO approving the rotation (include dates and specifics regarding resident training).
- Letter from Visiting Resident’s current Program Director indicating that a background check has been performed on Visiting Resident and there are no felonies on the background check report.
- Clearance Form for Visiting Resident (PDF)
- Application for Admission as a Visiting Resident (PDF)
- Memorandum of Agreement (completed by the Program Coordinator - PDF)
- Copy of Immunization Records (All visiting residents must meet all immunization requirements)
- HIPAA Confidentiality Agreement
- Copy of Medical Degree
- Copy of Social Security Card
- Copy of Temporary Kansas License
- Copy of Curriculum Vitae
- Copy of Driver’s License
- Copy of Health Insurance Card
- Copy of DEA Certification
- Copy of ECFMG Certificate or be eligible for Exchange Visitor (J-1) status if a Foreign Medical Graduate
- Copy of NPI Number
